Forms

• ARVDL Lab Submission Form - This is the general diagnostic testing submission form and should be used for all testing that does not require a specialized regulatory form.
• Pet Loss Form - This form must be submitted with all companion animal necropsy submissions. It is not needed for livestock and poultry animals.
• Rabies Forwarding Form - This form must be completed for any submission requesting rabies testing to be forwarded to the Arkansas Department of Health Rabies Lab.
• Commercial Poultry Submission Form - This form is reserved for testing corporate commercial poultry only. For privately owned poultry, please use the ARVDL Lab Submission Form.
• Client Confidentiality Waiver - This form must be completed by the submitter (vet, clinic, and/or billing party listed on the submission form) for each case in which the submitter wishes the ARVDL to send a copy of the report to a third party (someone who is NOT listed on the submission form).

PLEASE NOTE: All Equine Infectious Anemia (EIA or Coggins) and all livestock brucellosis testing requires a regulatory form. To order more regulatory forms, please contact the Arkansas Livestock and Poultry Division at (501) 823-1722.
